Quick answer
The CMS 2026 fact sheet sets the standard Medicare Part B premium at $202.90 a month and the annual Part B deductible at $283. Higher-income enrollees pay an income-related monthly adjustment amount (IRMAA). CMS says roughly 8% of Part B enrollees are affected.
Key takeaways
- The 2026 IRMAA table generally uses modified adjusted gross income from the 2024 federal tax return.
- IRMAA is individual: each Medicare enrollee in a couple can owe the adjustment.
- A qualifying life-changing event may support a request for a new decision using Form SSA-44.

Married filing separately: CMS publishes a separate schedule for some people who lived with a spouse during the tax year. Use the official CMS table before making a decision.
Why the table looks back two years
SSA generally uses the most recent federal tax return information the IRS can provide, which is commonly from two years earlier. For 2026 premiums, that usually means 2024 MAGI. SSA sends a notice explaining the income data and the premium determination.
When Form SSA-44 may help
If your income fell because of a qualifying life-changing event—such as marriage, divorce, a spouse’s death, work stoppage, or work reduction—review SSA’s current request-to-lower-IRMAA instructions and Form SSA-44. The request asks SSA for a new decision; approval is not automatic.
Part D has a separate IRMAA amount
The table above covers Part B only. Medicare Part D uses a separate income-related adjustment that is added to the drug plan premium. Check the CMS fact sheet and the determination notice for both parts.
